This situation had deteriorated since the last inspection in 2018. This was a focused inspection with emphasis on specific key lines of enquiry within the safe domain, the responsive domain and the well-led domain. Processes were in place to monitor performance. At Hope House, a dedicated member of staff contacted everyone who had been discharged from the service in the previous two weeks to ask their opinions. We found a good incident reporting culture where staff were clear on what to report and who they should report to. This meant that some patients were not receiving person centred care. Staff in teams felt they were effective in their jobs and patient surveys showed similar findings. There was a multidisciplinary approach to the delivery of care.
